Orlistat works through suppressing a special enzyme - named "gastrointestinal lipase". This enzyme breaks down triglycerides (a form of oil) in the digestive system. Without the lipase enzyme, oils in our diet are prevented from being hydrolyzed into absorbable small fatty acids and are eliminated in undigested form. Only very small volumes of Orlistat are absorbed by human body in the digestive system; the main effect is local lipase prohibition within the GI tract after an oral dose. The main path of elimination is through the faeces.
With the common prescription drug dose of 120 mg, and 3 times every day before meals, Orlistat successfully prevents about 35% of dietetic fat from being absorbed by body, and close to 20% at the standard over counter dosages of 60 mg/per meal. In the clinical drug test, higher dosages do not generate more powerful results.

The total weight loss accomplished with Orlistat may vary on different situation. In one year medical drug trials, about 36% to 55% of the research subjects achieved over 10% decrease in body weight, although not all of this weight was necessarily body fat. Between 17% and 25% achieved over a 25% decrease in body weight. The XENDOS study also found a 35% decrease in the incidence of type 2 diabetes. This is a very significant result.
